Ready for your exam?
It is very helpful if you bring any prior eye examination records to your exam. If you do not have old records you can bring the name and address of your prior eye doctor and we will request the records for you. Do not worry if you cannot remember where you last had an eye exam, we will start a new record for you. If you take medications please write out a list and bring this to your exam. If you wear contact lenses please bring the old boxes or lens packages so we can determine what you are wearing. If you have used up your last contact lenses and do not have any boxes or lenses remaining, we can re-fit you if the prior contact lens information is unavailable. Wear your contact lenses to the exam, we will provide a case and solutions to allow you to remove them once they have been evaluated. If you have diabetes we will always do a dilated eye exam. We will send a copy of your exam to your primary physician. Please bring the name and address of your primary physician and any other practitioner where you would like us to send an exam summary. If it has been a long time since your eyes were dilated we will suggest a dilated eye exam. There is no extra fee for this comprehensive service. If you are interested in a complete dilated exam but have to return to work and cannot be dilated, we can re-schedule the dilation for another day. While a dilated exam is more thorough, you will always have the choice of declining to be dilated, or to postpone dilation to your next exam. We prefer to do a comprehensive eye exam but understand that schedules do not always allow for dilation. |
